Carla is an out-of-network insurance provider. When you call, please have your insurance card ready so she can obtain the proper information for her insurance biller to verify your benefits.
Cash, all major credit cards (Visa, MasterCard, American Express, Discover, JCB, or UnionPay), Apple Pay, Android Pay, Chase QuickPay with Zelle, PayPal, Venmo, Health Savings Account (HSA), and Flex Spending Account (FSA) cards are accepted for payment.
If you do not show up for your scheduled therapy appointment, and you have not notified Carla at least 24 hours in advance, you will be required to pay the full cost of the session.
